Bloom Care Partners accepts inquiries and referrals from professionals and organizations serving children and adults with disabilities and older adults.
We welcome referrals from:

Some individuals can live independently without staff residing in the home but still benefit from regular or occasional assistance.
Semi-Independent Living Services can help individuals with intellectual or developmental disabilities maintain their homes, strengthen independent living skills, manage everyday responsibilities, and remain connected to their communities.
Support is designed around what the person can do independently and where additional assistance would be beneficial.
